The goal of Compass Connections is to help each child find their true north. Our goal is to ensure each child’s safety and well-being are paramount, with a focus on permanency – ensuring all children are safely placed with their forever family.

What We Do

Compass Connections is a large national provider of Case Management Services. Compass Connections provides various services focused on permanency, including intensive case management services that emphasize placement stability and safety and utilizes seasoned professionals trained to support the child-centered family unit through a culturally sensitive and trauma-informed approach.

Compass Connections provides case management services nationwide, based on referrals from our federal partner.

Placement Stability and Safety

Case Management Specialists work with caregivers to address challenges in parenting a child. This may include guidance about maintaining a safe home; supervision of the child; protecting the child from threats by kidnappers, traffickers and gangs; and information about child abuse, neglect, separation, grief and loss, and how these issues affect children.

Community Support

Education
Case Management Specialists assist the caregiver with school enrollment, including assisting with an alternative education plan for a child who exceeds the state’s age requirement for mandatory school attendance.

Medical Services
Case Management Specialists assist the caregiver in obtaining medical insurance for the child and locating medical providers who meet the individual needs of the child and the caregiver. If a child requires specialized medical assistance, the Case Management Specialist may assist the caregiver in making and keeping medical appointments and monitoring the child’s medical requirements.

Individual Mental Health Services
Case Management Specialists provide the caregiver with relevant mental health resources and referrals for the child. The resources and referrals will consider for the individual needs of the child and caregiver. If a child requires specialized mental health assistance, Case Management Specialists assist the caregiver in making and keeping mental health appointments and monitoring the child’s mental health requirements.

Family Stabilization/Counseling
Case Management Specialists provide the caregiver with relevant resources and referrals for family counseling and/or individual counseling that meet individual needs of the child and the caregiver.

Substance Abuse
Case Management Specialists will assist the caregiver in locating resources to help address any substance abuse-related needs of the child.

Gang Prevention
Case Management Specialists will provide the caregiver information about gang prevention programs in the caregiver’s community.

Resource Support

Immigration Proceedings
The Case Management Specialist will monitor and help facilitate the caregiver’s plan to ensure the child attends all required court proceedings and complies with all legal requirements.

Guardianship
If the caregiver is not a parent or legal guardian of the child, then the Case Management Specialist will provide the caregiver information about the benefits of obtaining legal guardianship of the child. If the caregiver is interested in becoming the child’s legal guardian, the specialist may assist the caregiver in identifying the legal resources to do so.

Legal Services
Case Management Specialists will assist the caregiver in accessing relevant legal service resources, including resources for legal advocacy and unresolved juvenile justice issues.

Trafficking Victim Assistance Programs (TVAP)
The Case Management Specialist assists qualified children in accessing case management services provided through Trafficking Victim Assistance Programs.

Other Services
Case Management Specialists may assist the caregiver and child with accessing local resources in other specialized service areas based on needs and at the request of the family.
