What We Do
Compass Connections is a large national provider of Case Management Services. Our work includes intensive case management that emphasizes placement stability and safety, assessed by seasoned professionals trained to support the child-centered family unit through a trauma-informed approach that is reflective of the population we serve.
Stability & Support
Education
Case Management Specialists assist the caregiver with school enrollment, including assisting with alternative education plans for children who need accommodations based on prior educational experience.
Medical Services
Case Management Specialists assist the caregiver in obtaining medical insurance for the child and locating medical providers who meet the individual needs of the child and the caregiver. If a child requires specialized medical assistance, the Case Management Specialist may assist the caregiver in making and keeping medical appointments and monitoring the child’s medical requirements.
Individual Mental Health Services
Case Management Specialists provide the caregiver with relevant mental health resources and referrals for the child. The resources and referrals will consider the individual needs of the child and caregiver. If a child requires specialized mental health assistance, Case Management Specialists assist the caregiver in making and keeping mental health appointments and monitoring the child’s mental health requirements.
Family Stabilization/Counseling
Case Management Specialists provide the caregiver with relevant resources and referrals for family counseling and/or individual counseling that meet individual needs of the child and the caregiver.
Substance Abuse
Case Management Specialists assist the caregiver in locating resources to help address any substance abuse-related needs of the child.
Gang Prevention
Case Management Specialists provide the caregiver information about gang prevention programs in the caregiver’s community.
